This service is designed to support people at the end of their lives who wish to spend their last days at home. It also seeks to provide support to their families and carers. It helps people to live as well as possible until they die, and to die with dignity. Our flexible team provides a fast responsive service that is adaptable to meet the needs of each patient: working to get a patient home as quickly and as safely as possible and providing a professional service that is tailored to their needs.
The holistic approach taken by RMS is person centred, i.e. that the client/ service user is the focus of care. In line with the gold standards framework of Investors in People (IIP), we look to communicate effectively with the clients/ service users and their carers, provide continuity of care wherever possible, and working closely with other agencies to ensure that care is carefully co-ordinated.
